Genesis Health Services Foundation Nursing Scholarship

Details:
•     Current College student in good academic standing, attending an accredited Iowa or Illinois college or university, who will have completed his/her freshman or sophomore year of college by June 30, 2011 and is seeking admission to, or has been accepted into, an undergraduate baccalaureate program in nursing.
Note: Individuals entering an associate’s degree program (e.g. community college), or BSN completion program are not eligible to apply.
• Resident within the Genesis Health System, bi-state service area: Clinton, Des Moines, Jackson, Louisa, Muscatine, or Scott Counties in Iowa or Henry, Mercer, Rock Island, or Whiteside Counties in Illinois.
• Minimum high school cumulative GPA of 3.0 and/or college cumulative GPA of 2.5.

Scholarship recipients will be notified by May 6, 2011

Scholarship terms:
1. The scholarship is awarded as a forgivable loan. Scholarship recipients will be required to sign a Forgivable Loan Agreement and a Promissory Note.
2. Recipients must remain a full-time student (minimum of 12 credit hours or equivalent status) in good academic standing until BSN is earned. Recipients who fall below full-time status for no longer than one semester shall not be deemed to violate this requirement.
3. Fifty percent (50%) of the loan will be forgiven if the nurse works for GenesisHealth System for one year. One hundred percent (100%) of the loan will be forgiven if employed full-time for two years. Recipients are expected to begin employment within three months following graduation. However, granting of a scholarship shall not be construed as an offer of employment.
4. If, for any reason, the student fails at any time to meet any of the requirements of the Forgivable Loan, the Loan must be repaid under the terms of the Promissory Note.

Application/Selection Process:
1. Application must be made on line at http://www.genesishealth.com/scholarships by deadline of March 18, 2011.
2. In addition to the on line application form, the following supporting documents must be submitted on line to scholarships@genesishealth.com prior to or on the deadline date of March 18, 2011. (Please include Applicant’s name and the words “Nursing Scholarship” in the subject line of the email when submitting documents.)
• One page essay explaining why the applicant wants to pursue a nursing career.
• Three letters of recommendation, such as from a professor, guidance counselor or clergy. One letter must be from an individual who has known the applicant for a minimum of three years and who is not a relative.
• Copy of acceptance letter into college or acceptance letter into a baccalaureate nursing program, if student already has been accepted into the program.• Official college and high school transcripts. (May be faxed or mailed per application page. ALL OTHER DOCUMENTS MUST BE EMAILED TO scholarships@genesishealth.com )
3. Upon completion of document submission, select applicants will receive a personal interview.
